NEW Campaign Finance Forms
Below are the forms that committees must use to register and file with the City Clerk Department. Per A.R.S. § 16-926(A), campaign finance forms are prescribed by the Arizona Secretary of State's office. Completed forms may be submitted electronically to the City Clerk's office by emailing them to CityClerk@yumaaz.gov
Statement of Organization - To be used to register a committee.
- Campaign Finance Reporting Schedules - To be used for campaign finance reporting.
- Termination Statement - To be used to terminate a committee.
- Financial Disclosure - To be used by elected officials and candidates.
